Available in stores along side the actual Gold Mine Icicles, the form allows you to order a Tee Shirt and Cap from a number of MLB stars of 1961 including Jim Lemon, Norm Larker, Willie Mays, Norm Siebern, Ernie Banks, Pete Runnels, Gus Bell, Harvey Kuenn, Roger Maris, Ron Hansen, Rocky Colavito, Nellie Fox, Dick Groat, Ernie Broglio, Warren Spahn, Robin Roberts, Mike McCormick and Jim Perry. The front of the order form pictures Pittsburgh Pirates Dick Groat along with pictures of kids wearing shirts with pictures of Dick Groat and Roger Maris! Measuring 5.5 x 4.25 inches, the yellow order form has some discoloration and holes from the original staples that held the form together in a pad with other forms (see photos).
